〔成分〕

混合甲磺酸氢化麦角碱

片剂：内含1．0mg，1．5mg或4．5mg本品^*。

口服液(1ml=20滴)：内含1．0mg本品。

口服液(1ml=20滴)：内含3．0mg本品。

安瓿(1ml)：内含0．3mg本品。

*本品由二氢化麦角考宁、二氢化麦角汀和二氢化麦角隐亭(二氢基-α-麦角隐亭与二氢基-β-麦角隐亭之比为2：1)的等分甲磺酸盐组成。

〔性质〕

动物研究表明，本品可改善大脑神经传导，并且有证据表明，本品对多巴胺及5-羟色胺受体有兴奋作用，对α-肾上腺受体部位有阻断作用。本品可改善受损大脑的代谢功能。该效果可反映在脑电活动的变化上，也明显地表现在脑电图的功率谱上。

人体实验研究也证实了对脑电图的这种有利作用，也发现本品能缩短大脑血液循环时间。临床对照试验证明本品可改善许多精神退化症状，特别是在自理、社交行为、情绪状态及精神表现等衰老有关的症状。本品对头部血管紧张度也具有稳定作用，因此它可以预防偏头痛。

〔适应证〕

明显与衰老有关的精神退化症状及征象：头晕，头痛，精神难于集中，定向力障碍，记忆力受损，缺乏主动性，情绪抑郁，孤僻及日常生活和自理困难等。

急性脑血管疾病。

偏头痛及血管性头痛(仅用于预防性治疗)。

〔剂量〕

口服：3～6mg／d，分数次，最好在饭前服用。

若为每日一次的剂量，每次1片(4．5mg)，或1．5ml(30滴)浓度为3mg／ml的口服液，建议早饭前服用，或遵医嘱。

对于精神退化或偏头痛病人，症状常逐渐缓解，3～4周后明显见效。需要时可延长疗程(3个月或更长的时间)，或遵医嘱。

非肠道给药的方法：急性脑血管异常情况下(特别是与高血压有关时)，开始时除口服治疗外还需要肠道外给药。每日1～2次静脉滴注或缓慢静脉注射0．3mg(1ml)本品(溶于20ml葡萄糖或生理盐水中)。或者肌肉注射或皮下注射0．3mg(1ml)本品，每日1至数次。

〔禁忌证〕

已知对本品产生过敏反应者禁用。

〔注意事项〕

有严重心动过缓时需小心。因为可能发生血压下降，故注射后应检查血压。

注意保存，防止儿童触动。

〔副作用〕

偶有鼻塞，一时性恶心和胃部不适，但如将本品与食物共服，常可预防上述现象。绝大多数情况下不采取特别措施，副作用即可消失。

Composition

Co-dergocrine mesylate*(BAN)

*Composed of equal parts of the mesilates of dihydroergocormine，dihydroergocristine and dihydroergocryptine(dihydro-α-ergocryptine and dihydro-β-ergocryptine in the proportion 2 to 1)．

Properties

Animal studies indicate that Hydergine modifies cerebral neurotransmission，and evidence is avaible for a stimulant effect on dopamine and serotonin receptors and for a blocking effect at α-adrenoceptor sites．It improves impaired cerebral metabolic function，an effect which is reflected in changes in the electrical activity of the brain，notably in the power spectra of the electroencephalogram．

This beneficial effect on the EEG has been confirmed by experimental studies in man．Hydergine has also been found to shorten cerebral circulation time．Controlled clinical trials have shown that Hydergine is effective in improving many of the symptoms of mental deterioration，especially age-related symptoms in the areas of self-care，social behaviour，emotional state and mental performance．Hydergine also has a stabilizing effect on the tone of cranial vessels，and this accounts for its prophylactic effect in migraine．

Indications

·Symptoms and signs of mental deterioration notably those related to ageing：dizziness，headache，poor concentration，disorientation，impaired memory，lack of initiative，mood depression，unsociability，difficulties with daily living activities and with self-care．

·Acute cerebrovascular disease．

·Migraine and vascular headaches(Preventive treatment only)．

Dosage

Orally：3-6 mg daily，in divided doses，preferably before meals．

For once-a-day dosage 1 tablet of 4．5mg or 1．5 ml(30 drops)of the 3mg／ml oral solution are recommended before breakfast unless otherwise prescribed by the physician．

In patients with mental deterioration or with migraine，the alleviation of symptoms is usually gradual and becomes manifest after 3-4 weeks．Prolonged therapy(3 months or more)is indicated and the course of treatment may be repeated as required．

Parenterally：

In acute cerebrovascular disorders(especially when associated with hypertension)parenteral therapy is indicated initially in addition to oral treatment：0．3mg(1 ml)Hydergine by i．v．drip or slow i．v．injection(in 20 ml dextrose or saline)once or twice daily．Alternatively，0．3mg(1 ml)may be given intramuscularly or subcutaneous once to several times daily．

Contraindication

Known hypersensitivity to the drug．

Precautions

Caution is required in the presence of severe bradycardia．

Blood pressure should be checked following parenteral administration，as a drop in blood pressure may occur．

Hydergine should be kept out of the reach of children．

Side effects

Nasal stuffiness；transient nausea and gastric upsets may occur occasionally，but are usually prevented by taking the drug with food．In the majority of cases side effects disappear without specific measures being taken．
